Once you have called us and scheduled your appointment in either the NJ or NY office, please download the Immigration Evaluation Forms in your preferred language of either English or Spanish from our website at www.evolve-ps.com. You will be providing detailed information about your history that includes family history, education history, occupational history, financial history, medical history, trauma history, relationship history, psychiatric history, country conditions, etc.
You can bring the completed paperwork with you to your appointment. If you are unable to access this paperwork ahead of time, it will be provided to you at the time of your appointment. You will also complete several self-report psychological assessment measures at the time of your appointment.
A comprehensive, written report will be generated outlining important background information, current stressors and functioning, clinical conclusions and recommendations.
